The LPN Care Coordinator is responsible for patient care within the setting of the clinic environment. The Care Coordinator position also assist patient with managing their health care needs remotely (via telephone, or video calls). They are trained and capable of performing patient care for the age groups newborn to 105 years old.
The LPN Care Coordinator reports directly to the RN Director of Clinic Nursing.
Take vital signs; obtain medical history, gives injections and immunizations as ordered by the clinic providers.
Maintain required logs and information needed for quality control.
Performs Medicare Annual Wellness (AWV); implementation and oversight of a Medicare Chronic Care Management and Transitional Care Management Programs.
Work quality measures for improvement of patient care measures for MIPS/MACRA, HEDIS, individual payer source reports, registries in Meditech, and other resources to improve quality measures for the Medical Group, provider and patients.
Collaborates with clinic providers and staff to strengthen data quality, collection, analysis, and reporting of health information.
LPN graduate of Accredited School of Nursing. Associate Degree required;
Bachelor’s Degree preferred.
Must have valid, current license by the Missouri State Board of Nursing
Current Basic Life Support (BLS) certification/Cardiopulmonary Resuscitation (CPR)Required
3-5 years’ experience in a clinical nursing area. Prefer physician office or case
