Position: Population Health Advanced Practice Provider

The Population Health Advanced-Practice Provider (APP) is a full-time position within the Quality Informatics and Specialty Services Division. The position is responsible for interacting with patients and working to achieve organizational quality objectives, including those of maintaining continuity of care, prevention, providing higher quality health care services (like Medicare annual wellness visits), improving patient safety, and improving patient satisfaction.
