To work in coordination with the primary care teams to facilitate Annual Wellness Visits and Health Care Maintenance. There will be some crossover with primary care responsibilities as well, but this position is being posted as a predominantly Annual Wellness Visit position.

Looking for providers who are dedicate to the community, and have an interest in serving our older population. Must understand compassionate care, have good communication skills, and work well with a team.

Experience in completing Annual Wellness Visits is helpful, though (for both newer and experienced providers) designated support, protocols, and trainings are in place.
