This is a full-time remote opportunity with a Monday - Friday schedule, 9am-5pm with some Saturdays required. Starting pay is $20/hour.
Iris Healthcare, powered by Aledade, Inc., is searching for highly compassionate, outgoing, and motivated Outreach Specialists to join our growing, diverse team. In this role you will use your active listening skills, perform outreach to patients on the phone to enroll them in our Comprehensive Advanced Care Planning (CACP) program, and you will provide education to patients about services that are available to them. The ideal candidate will have excellent verbal communication and documentation skills combined with a background in patient outreach, case management, client management, or call center environment.

Advance care planning is defined as a process that supports individuals in understanding and sharing their values, goals, and preferences for future medical care. These care decisions are then formally documented within an Advance Directive; a forward-thinking legal document that empowers individuals, their loved ones, and their care teams to make the best advance care planning decisions moving forward. Position Requirements

  • Client-focused mindset, including the ability to put clients at ease and maintain composure while addressing stressful, challenging healthcare topics
  • Meet goals using company metrics for outreach while delivering high customer service and satisfaction rates
  • Ability to prioritize performance metrics and balance daily workload
  • Conduct a large volume of outbound phone calls daily to new members
  • Document conversations and activity in company database
  • Maintain confidentiality of sensitive client information

Preferred Experience and Qualifications

  • 2-4 years of experience in client management, enrollment, patient outreach or similar role
  • Mission-driven sales or outreach experience
  • Associates degree, Bachelor's degree
  • Work in healthcare currently or previously
  • Background in motivational interviewing or related
  • Advanced care planning knowledge or familiarity with end-of-life care
