Iris Healthcare powered by Aledade, Inc. is looking for three Senior Facilitators to promote from within. These are hourly positions, 30-39 hours per week. The ideal candidates are highly self-motivated, outgoing, compassionate, mission-driven professionals within our CACP Facilitation team. The Sr. Facilitator is a high performing Facilitator that will partner with the Manager, Facilitation by providing administrative support and serve as a subject matter expert for the Team.

Duties/Responsibilities

  • Maintain a current Patient list
  • Approval of Patient Documents
  • Listen to portions of calls for administrative reasons (verify content of documents or help Supervisor prep a training)
  • RingCentral/Vida technical triage/support
  • Resource finding
  • Team Chat Monitoring
  • Escalating clinical issues/questions to Manager
  • Provide feedback to Leadership related to trends and training needs
  • Performs other related duties as assigned.

Required Skills/Abilities

  • Licensed RN or Social Worker is a must
  • Strong verbal and written and communication skills
  • Ability to coach and provide effective feedback
  • Excellent organizational skills and attention to detail
  • Strong time management skills with a proven ability to meet deadlines
  • Demonstrated consistency in high quality Facilitations and documentation
  • Ability to collaborate with the Team and Leadership
  • Regular and predictable attendance
  • Experience with Documentation Review and/or other quality review processes

Education and Experience

  • Must meet the minimum qualifications of a Facilitator
  • At least 6 months CACP Facilitation experience (post onboarding)

Physical Requirements

  • Prolonged periods of sitting at a desk and working on a computer.
