Location: Kazan Memorial Clinic
Position: PHYSICIAN ASSISTANT/NURSE PRACTITIONER
A. Required to perform a range of basic medical care services under supervising physician. These services include: patient histories, physical examinations, performing routine diagnostic services, recommending treatment - for both acute and chronic disease states. Interacting with other professional personnel in the diagnosis and treatment of the patient. Also proficient in reading x-rays of the chest and extremities, suturing, offering limited psycho-social counseling, performing lab tests and phlebotomy for diagnostic purposes. Also required to provide emergency medical care such as/but not limited to: intubating, IV fluids, catherizing, triage, control bleeding, BLS, ACLS, including the use of thrombolytic according to protocol when located in Bicknell.
The role of the provider in this position is quality care of routine medical problems such as HTN, CHF, diabetes, renal disease, basic mental health issues and referral of medical complication beyond the incumbent’s scope.
B. Is responsible for maintaining patients’ medical records, including SOAP formatted records of each encounter, problem list, referrals and outcome of referrals, laboratory and x-ray results and report results to patient, phone consultation records, consent for treatment, release of records information forms. Also, complete patient billing form for accuracy in billing patient.
C. Attend staff meetings. Attend Quality Improvement Committee meetings monthly as requested, participate in ongoing Quality Improvement and Peer Review, ACO with Aledade and UDS measures for HRSA Grant.
D. Provide on-call emergency medical coverage as per schedule.
E. Participate in special clinical events, i.e., special clinics, health fair, etc., etc.
F. Other duties as requested by Medical Director, Board, or Administrator.
Reporting relationship:
Supervisors: 1. Medical Director, 2. Administrator
Supervisory responsibility: Oversees medical assistant staff as required by medical director.

QUALIFICATIONS:
- Must be State and National Certified as P.A.-C./APRN
- Must keep certification current
- Must keep State of Utah, Dept. of Commerce License current.
- Must keep ACLS certification current
- Must work well with other staff
- Must have excellent patient “bedside” manner
- Must be capable of providing health care while under pressure
- Be well groomed and dressed
- Must be capable of ambulating stairway carrying 25 lbs.
