Position: Patient Navigator
Full Time
Monday to Friday
8am – 4pm
Medical Background/Certification preferred.
Pay range is $19.00-$23.00

Job Duties:

  • Main job focus is to assist with the communication and direction from Provider to Patient. Assisting the patient in navigating the healthcare system by way of orders, connection with healthcare partners, and communication to Providers.
  • Monitor Lab Portals o Upload results to chart and send to ordering provider.
  • Process any necessary follow up after provider talks to patient.
  • Monitor Radiology Portal
  • Upload results to chart and send to ordering provider.
  • Process any necessary follow up after provider talks to patient.
  • Process all Lab and Radiology orders/tasks from providers.
  • Call patients with details of where orders were sent as well as phone numbers.
  • Process all DME orders/tasks from providers.
  • Call patients with details of where orders were sent as well as phone numbers.
  • Take patient calls regarding updates and communicate with company when necessary.
  • Ring Central Voicemails
  • Transcribe into Tasks
  • Refill Line
  • Clear by end of day (anything up until 3:30pm)
  • Check Inspect for all CS.
  • Refills of approved medications sent on Provider behalf
  • Prior Authorizations
  • Process as they come through fax/email/patient calls.
  • Create tasks in Aprima to update as prior authorization is worked.
  • Call patients with final determination.
  • Work with provider as needed via Aprima task for clinical information.
  • Complete tasks sent by Providers.
  • Urgent tasks responded by end of day.
  • Communicate efficiently and effectively with Providers and Executive Director of Clinical Operations
