Join Aledade as a Practice Transformation Specialist (PTS) within our Flex Field Team. This is a traditional PTS role, but with more frequent changes in assignment, and reporting to the Performance Team while coordinating with the existing Market Leadership (when there is one).
Examples:
-New market development, where practices have signed one or more contracts but the planned new field team has not started in their roles.
-Augmentation where sudden growth has occurred, helping with launch as the current field teams reconfigure or expand.
-Augmentation where sudden field team deficit occurs, as the current team interviews and hires up.
-Temporary assignments with known start and stop dates
Being a PTS means you are an expert in change management, in the transformation of our partner-practices’ workflows so that they move from fee for service dependence to embracing value based care. Project management skills are valuable, as you will help practices with workflow enhancements that drive outcomes in the practice that improve care and reduce costs. You should be an expert in the Aledade App and how our practices can optimize their use of it. You should feel very comfortable in the use of data to drive incremental change. The mission is to change the landscape of healthcare delivery for each practice. The number of practices supported will vary based on Aledade needs and may at times be more or less than common PTS: practice staffing ratios. This role also differs from the traditional PTS role in that the relationships built with practices are no less important, but they are shorter-term.

Major Duties:

  • Launch and onboarding of new practices
  • Evaluating practices on workflows and coaching for improvements and efficiencies
  • Help inform Aledade in the development of scalable tools for PTS work. Share best practices with teammates across Aledade via PTS Roundtables and other forums.
  • Project management
  • Strong relationship building skills
  • Exporting data and running reports
  • Problem solving for efficient and effective progression of initiatives
  • Partner with Performance Team leadership and Market leadership (ED, RMD, etc) when present on project-specific timelines
  • Communicate areas of concern, needed tools, or barriers to achieving goals
  • Assist in the preparation of proposals, reports, updates, and summaries
  • Identify opportunities for improvement in the operations

Required Qualifications:

  • A degree in nursing or social work experience in a clinical setting is helpful, as is experience working as a primary care office manager. Graduate of an accredited college or university in health technology or information technology-related program accepted.
  • Experience working with MSSP and non-MSSP contracts
  • 20% required travel
  • Excellent oral and written communication skills (i.e., public presentation skills), organizational and project management skills, and team-building skills
  • Familiarity with process mapping and work-flow analysis tools
  • Experienced with Electronic Health Records (EHR) for clinical processes - preferred
  • Excellent computer skills and willingness to learn additional software applications
