Job Description Summary

The Registered Dietitian (RD) delivers high-quality, evidence-based nutrition care and education to support apree’s Nutrition Education and Weight Management programs. As a key member of the clinical care team, the RD collaborates with primary care providers, health coaches, behavioral health clinicians, and other members of the patient’s care team, to engage patients in their health and well-being. Through individualized nutrition therapy and patient-centered care planning, the RD helps patients achieve sustainable behavior change, reduce health risks, and optimize outcomes.

How will you make an impact & Requirements

Essential Functions:

Nutrition Education & Counseling

  • Provides individualized Medical Nutrition Therapy (MNT) for patients with diverse needs, focusing on weight management, cardiometabolic risk reduction, and healthy lifestyle behaviors.

  • Delivers nutrition education in individual and group settings, using motivational interviewing and evidence-based counseling strategies to facilitate behavior change.

  • Develops patient-specific nutrition prescriptions and interventions that are realistic, culturally sensitive, and aligned with patient goals.

Weight Management Support

  • Partners with RN care managers, health coaches, and providers to deliver care within apree’s structured weight management program.

  • Conducts comprehensive nutrition assessments and creates individualized care plans.

  • Supports patients on weight loss medications, such as GLP-1 medications, with nutrition-focused strategies to maintain outcomes.

Care Coordination & Documentation

  • Actively collaborates with the care team to align recommendations and adjust plans based on progress and outcomes.

  • Documents all care encounters in Athena EHR accurately and in a timely manner, linking care plan goals to program-specific health concerns.

Program & Quality Improvement

  • Contributes to program development by creating and updating patient-facing education materials and provider job aids as needed.

  • Participates in team meetings, care center onboarding, and program quality improvement initiatives.

  • Provides external presentations on a variety of topics and initiatives

Patient Engagement

  • Uses a variety of outreach methods (telephonic, video, portal messaging) to engage patients and support sustained participation.

  • Provides encouragement and accountability to help patients achieve their goals.

Qualifications:

Required:

  • Registered Dietitian (RD/RDN) credential

  • Bachelor’s degree in nutrition, dietetics, or related field

  • Minimum 2 years of experience providing MNT in an ambulatory or outpatient setting

  • Experience with weight management, cardiometabolic risk reduction, and lifestyle counseling

  • Strong virtual care skills and comfort delivering care through telehealth platforms

  • Proficiency in EMR documentation (Athena experience preferred) and Google Workspace

Preferred:

  • Master’s degree in nutrition or public health

  • Experience working within an integrated primary care model

  • Group facilitation experience (classes, workshops)

  • Familiarity with GLP-1 therapy and discontinuation support strategies

  • Integrative and functional nutrition experience

  • Bilingual or multilingual

Compensation: $27.00/hr. - $34.00/hr. & bonus eligible
